Unique Insurance Service, Inc., doing business as Agency Service Bureau (“ASB,” “we,” “us”), 737 Southpoint Blvd, Ste F, Petaluma, CA 94954, CA License #0381168, operates this website for appointed and prospective independent insurance agencies. This notice describes personal information and nonpublic personal information we collect through this site and in connection with an agency appointment, a quote, or policy service.
Information we collect
We collect only what you choose to send, plus limited technical information our host records so the site can run.
- Identifiers and contact details. Name, agency name, email, and phone number from the contact, appointment, and quote forms.
- Commercial and professional information. The subject and message you write, including states, volume, or a description of a risk.
- Insurance and customer information. When an appointed agency emails us a completed application or quote form, that file may include an insured’s name, address, date of birth, driver’s license number, Social Security or tax identification number, property details, and coverage information. We do not ask for that information on the website forms themselves.
- Internet activity. Our website host may log the IP address, browser type, and pages requested. We do not use advertising cookies, analytics pixels, or cross-site tracking.
We get this information from you, from the agency that submits a risk, and from the carriers and systems used to quote or service that risk. We do not buy personal information.
How we use it
- Answer appointment, quoting, and service requests.
- Set up and administer an agency appointment, including commissions.
- Route a risk to an appropriate market and communicate about that account.
- Keep records required for insurance licensing, audits, and claims.
- Protect the site from spam and misuse. The forms include a hidden field for that purpose.
- Meet a legal, regulatory, or law-enforcement request.
We do not use personal information to train public AI models, and we do not use sensitive information such as a Social Security number or driver’s license number to infer characteristics for advertising.
Insurance privacy
ASB is an insurance licensee. Nonpublic personal information we receive about a person in connection with an insurance transaction is used to quote, place, and service coverage and to meet carrier and regulatory duties.
We do not share that information with nonaffiliated companies so they can market their own products to you or to an insured. Because we do not share for that purpose, there is nothing to opt out of. We do share information with the insurance companies and administrators needed to underwrite, bind, and service a policy, with our service providers, and when the law requires it.
Who receives information
We do not sell personal information. We do not share it for cross-context behavioral advertising.
- FormSubmit (formsubmit.co) delivers website form submissions by email to our staff. A submission includes the fields on the form and the page address.
- Our email and website hosts store messages and serve this site.
- Insurance companies, wholesalers, and administrators receive submission information when we pursue a quote or service a policy.
- Professional advisors and regulators receive information when needed for licensing, an audit, a legal claim, or a required report.
EZLynx, carrier portals, and other sites we link to are separate. Their privacy notices apply once you leave this site. Clicking a staff name opens your own email program. That does not send information to us until you send the message.
How long we keep it
We keep information for as long as we need it to respond, to service an appointment or policy, to meet insurance recordkeeping rules, and to resolve a dispute. We then delete or de-identify it unless a longer period is required.
Your choices and rights
Depending on where you live, you may have the right to know what we hold, to receive a copy, to correct it, to delete it, to appeal a denial, and to use an authorized agent. We will not discriminate against you for making a request. We may still need certain information to appoint an agency or to place coverage, and we may deny deletion when an insurance, tax, or legal duty requires us to keep the record. We will tell you if we do.

We may ask you to confirm that you are the person, or the agency, the information is about. An authorized agent should include proof of authority. We respond within 45 days. If we need more time, we will tell you, and we will not take more than 90 days in total.
If we deny a request, you may appeal by replying to that email within 45 days. We will answer the appeal in writing within 45 days and, where the law requires it, tell you how to contact your state attorney general.
Children
This site is for insurance agencies. We do not knowingly collect personal information from anyone under 16. Contact us if you believe we have, and we will delete it.
Security
We use reasonable administrative and technical safeguards appropriate to a wholesale insurance office. No website or email transmission is completely secure. Do not include a Social Security number, bank account, or payment card number in a website form. Use the published applications and our usual secure submission process for that information.
Changes
If we change this notice, we will post the new version on this page and update the date above. The version on this page is the one that applies to this website.
